Extended Care and Rehabilitation

About the Program

The Extended Care and Rehabilitation Program at VAMC Togus is composed of four areas —

  • Physical Medicine and Rehabilitation
  • Nursing Home Care Unit (NHCU)
  • Home Based Primary Care (HBPC)
  • Geriatrics & Extended Care Services (G&EC)

Services Offered

Physical Medicine and Rehabilitation (PM&R)

PM&R services include —

  • occupational therapy
  • physical therapy
  • speech therapy
  • kinesiotherapy
  • specialized services to veterans in need of rehabilitation following amputation, stroke, traumatic brain injury, and spinal cord injury

Nursing Home Care Unit (NHCU)

The NHCU is a hospital-based transitional care unit. Most patients require extended rehabilitation and care following surgery and/or lengthy hospitalizations before returning to independent living.

Home Based Primary Care (HBPC)

HBPC consists of programs that manage care in veterans homes and community settings. Services include —

  • home-based primary care
  • home telehealth care
  • adult day health care
  • community nursing home care
  • skilled home care
  • home hospice care
  • home IV therapy

Geriatrics & Extended Care Services (G&EC)

  • geriatrics primary care
  • geriatric and geropsychiatric consultations
  • geriatrics evaluation and management
  • inpatient/outpatient
  • special care dementia unit
  • contract nursing home care program
